419 Troop / Patrol Trips

Scout Info	This on-line form is used to update the Troop records with new and revised information for Scouts, Parents and other Adult Leader. Once the form is completed it is emailed directly to the appropriate committee individual.
Permission Slip	Each 419 Troop and Patrol outing requires a permission slip for both adults and boys. The permission slip is an integral part of the T-Minus program and needs to be adhered to. Permission slips are due no later than T-3 of an event.
Tour Permit	This permit must be filled out prior to a trip and must be signed by a committee member as well as the tour leader. Don't forget to review the Trip Policy of the Troop.
Patrol Meal Planning Worksheet	This worksheet can be used by each patrol to record assignments for each campout.

Action Packer Checklist	This worksheet should be used by each patrol to make sure their Patrol's action packer is not missing any items. Remember that Patrols are responsible for the contents of their action packer.

Health Forms

Youth & Adults < 40	Class 1 is to be filled out annually for all participants. This is for any activity that doesn't exceed 72 hours. Class 2 is required to be filled out every 36 months for activities longer than 72 hours (Summer Camp, Long Backpack trips, etc.)
Adults 40 or older & for High Adventure	Must be filled out annual for adults over 40 or for adults and boys before participating in high-adventure activities.
Adult Training Award	Adult leader training award application.
